This survey contains questions about the current or potential application of an electronic health record system (EHRS) in your practice. For the purposes of this study, an EHRS is a unified repository of healthcare information that is accessible from multiple locations. It captures episodic and longitudinal patient interactions, and functions as the clinicians' primary resource during the provision of patient care. The data captured in the EHRS may also be used for other purposes such as clinical research.
Most questions in this survey will be relevant for you whether or not your practice has adopted an EHRS. Your participation in this survey is voluntary and any personal information you choose to provide will be kept confidential.
You will first be presented with a consent form to which you must agree if you wish to participate in the survey. The survey consists of 4 parts and should take approximately 15-20 minutes to complete. Please read each question carefully. There are no right or wrong answers. Your feedback is very important.
The survey has been developed with input from the staff at the ACP Center for Practice Innovation (CPI). As a small token of our appreciation, we have arranged for a book on EHRS implementation to be sent to you upon completion of the survey.
