Informed Consent
You are invited to participate in a research study that will investigate parents’ attitudes, feelings, and behaviors in response to situations where their child shows signs of anxiety.
If you are not 18 years of age or older, and do not have a child between the ages of 5 and 13 please do not complete this survey. This study is being conducted by Caitlin Burditt, MA, as her dissertation in partial fulfillment of a Ph.D. in
clinical psychology at the University of Rhode Island.
If you decide to take part in this study, the following questionnaire will ask you a
number of questions about your emotional and behavioral responses during times when your child is anxious. In addition, we will ask you about the importance of factors that may influence your views of your child’s anxiety as well as your actions when they
are anxious. You will also be asked to provide some background information about yourself and your child. This survey will take approximately 25-30 minutes to complete.
