CREATE Health Scholars Team Leader Application Checklist:

o Complete Team Leader Application on Zoomerang. You will not be able to save as you go, so you must have all of the following materials ready to submit before beginning. For example, you will want to prepare your regional health challenge statement and assemble your reference contact info in advance.

The CREATE Health Scholars program will be held at Regis University and the University of Colorado-Anschutz Medical Campus from:
June 4 through 28, 2012 and July 5 through 27, 2012.
Applications will be accepted through April 30, 2012.
